San Isidro ISD Required Athletic Forms Now Available!!! (updated July 2022)
Required athletic forms for the 2023-2024 school year are now available for download.
- Student-athletes enrolled in grades 6th - 12th MUST complete a Medical History Form and all required documents prior to participating in ANY practice, game, in-season and off-season athletic program.
- All incoming 9th graders, 11th graders, and first-year Jr. High participants MUST have a sports physical exam completed by a physician of choice prior to participating in ANY practice, game, in-season and off-season athletic program.
Any Jr. High or High School student-athlete who wishes to participate in athletics during the 2023-2024 school year must print and have the following forms signed and submitted to the athletic department before participating:
